To qualify for membership, you must live or work in Wood, Marathon, Clark, or Portage County, Wisconsin, or have an immediate family member that is a
First Choice Credit Union member.

1. Please complete the following online Secure Account Application Form, and click ‘submit’.

2. Once we have received your application, we will contact you to schedule an appointment to complete the account opening
process AT the Credit Union.

3. Please bring a photo ID for primary & joint member (if applicable) AND proof of current address if your drivers license address is not current.
Documents we accept for proof of address are:

a. Recent Utility Bill b. Recent Paystub
c. Recent Lease or Mortgage d. State Issued ID Card
e. Income Tax Returns f. Unexpired Passport

4. Membership with First Choice Credit Union requires a $5.00 initial deposit into your share (savings) account. Please bring this with you to
your appointment. You need to maintain a $5.00 balance in this account at all times.
